Albert has been a healthcare executive for over 27 years. He has a demonstrated history of working in large practices and large healthcare organizations. His knowledge of operations, finance, and business management within healthcare has led to the development and growth of several medical practices.
Albert is the Vice President of Development for a multi-specialty physician practice where he helped develop the connected care department which includes chronic care management, remote patient monitoring, principal care management, transitional care management, telehealth, and annual wellness visits. Through this experience he witnessed first-hand the positive outcomes for both the patients and practices.
He is passionate about supporting the success of care and operations within practices. Because of this, he has partnered with Dr. Nicolas Chronos to develop a streamlined, advanced, and effective chronic care management program.
Previously, Albert was the AVP of Cardiology Practice Services where he oversaw all operational and financial aspects of Gwinnett Health System. In addition, as CEO of CardioVascular group, Albert oversaw day to day operations, cash management, budgeting, banking and all other aspects of running a 30 million dollar medical practice.
Chief Medical Director
Dr. Nicolas Chronos, managing partner of Lake Country Medical Group and Connected Care Institute, is an internist and interventional cardiologist well known for his groundbreaking research in heart disease treatment. In addition to his internal medicine expertise, he specializes in general cardiology, coronary artery disease management and heart failure management. He is also an expert in cardiac regenerative medicine, cardiovascular medical device assessment and translational cardiovascular medicine.
Dr. Chronos began developing a robust Chronic Care Management program in his own practice, Lake Country Medical Group. He realized that the program he developed could benefit other practices as they care for patients with chronic conditions.
Dr. Chronos received a bachelor’s degree in medicine and surgery from the Royal Free Hospital School of Medicine at the University of London in 1987. He then trained in cardiology and interventional cardiology at the Royal Brompton National Heart and Lung Institute in London.
In 1992, Dr. Chronos was awarded a British Heart Foundation International Fellowship and moved to the United States to continue his interventional cardiology research at Emory University School of Medicine under the direction of Dr. Spencer B. King, III. He was named director of research at the Andreas Gruentzig Cardiovascular Center at Emory University Hospital in 1997.
In 1999, Dr. Chronos joined Atlanta Cardiology Group, where he developed and served as CEO of the Saint Joseph’s Translational Research Institute until 2012, when he left to form Cardiology Care Clinics. Dr. Chronos has held academic appointments at Duke University and is currently an adjunct professor of medicine and cardiology at Stanford University.
Dr. Chronos is a fellow of the American College of Cardiology, the European Society of Cardiology and the Royal College of Physicians of London. He has published several books and more than 200 peer-reviewed articles.
Chief Pharmacy Officer
Gene has been a pharmacist for 19 years. He has held administrative and leadership positions which include leading the execution of multiple patient facing health care initiatives. He has initiated the deployment of large-scale healthcare screenings, vaccine education and administration, as well as patient medication consultation.
Gene is passionate about ensuring patients receive adequate counseling. Because of this he has been instrumental in setting up the Medication Reconciliation program as part of Connected Care Institute. The Medication Reconciliation program focuses on looking for duplication of therapy, drug interactions for prescription and OTC pharmaceuticals and nutraceuticals, addresses side effect and medication effectiveness concerns for patients, and assists patients in finding the most cost effective solutions to obtain their medications
Having a pharmacist on the chronic care management team enhances patient care by optimizing medication therapy, improving medication adherence, preventing errors, promoting patient safety, and facilitating collaborative and patient-centered care. Gene's expertise in medication management is instrumental in achieving better outcomes for patients with chronic conditions.
